Convenient walk-in service at all Sam’s Health Mart Pharmacies
- Employee presents their benefit card or voucher at pharmacy
Pre-arranged clinic at workplace
- Our pharmacists come to worksite and administer
- Available for 25 employees or more
- Scheduled at your convenience between October and January
- One month notice required for cancellations
- Minimum commitment required
Restrictions
- Ages 12 and above - no prescription needed
- Ages 4 to 12 - prescription required authorizing pharmacist to administer (telephone orders from prescriber accepted)
- Under 4 - not available
Billing
- Claims direct billed to employer at the end of each month
